If you are a candidate who is running for a Congressional Office, please download the Federal Questionnaire here.  Please complete the form and email it to the address listed on the cover page of the questionnaire.

If you are a candidate who is running for a Local or State Office and would like to be considered for endorsement by your local NWPC CA chapter, please download the State Questionnaire here and the Local Questionnaire here.

Candidates who were previously endorsed by NWPC SGV and are running for re-election to the office for which they were endorsed may sign a reaffirmation letter rather than completing the endorsement questionnaire again.  Please contact your local caucus representative for the re-affirmation letter at nwpcsgv@gmail.com
